Good Afternoon:
I have two questions regarding A5512 and A5513 custom inserts:
My first question is in regards to the guidelines that my fellow
practiitoners follow when deciding to use A5512 heat-molded inserts or A5513
custom inserts. While specific criteria has been established regarding when
we are allowed to bill for these inserts; it is difficult to find
guidelines for choosing one style over the other.
My second question involves your documentation regarding these devices. What
type of practitioner (not physician) documentation do you do for these
devices? Does it differ if you are providing the A5512 inserts or the A5513
inserts?
