I have a 7 yr old patient with JRA, underlying Downs' syndrome. When she was 3 yrs old, she stopped walking due to the severe JRA onset. She currently presents with knee extension to 120 degrees only, and one of her feet tends to valgus. Dynasplints have failed in the past, she could not tolerate them. She cannot stand on her own, her knees are too flexed and she is weak. She is starting PT and the goal is ultimately for her to walk again. Rx reads for bilat KO's, unilateral foot extension. I intend to recommend bilat KAFOs with molded inner boots due to the tendency toward valgus in this population, as we stretch the structures behind the knee including the gastrocs, and for a myriad of other good reasons.

My question is, componentry. I have tapped all my resources. I believe that the 6 degree increments on the Becker adjustable extension drop lock joints will be too difficult to progress her with. So I started looking elsewhere. Has anyone used the Scott Orthotic Labs Dyna-Lock Ankle joint (see p. 283 new KR catalog/p. 439 of KR catalog 16)? What I like about it is that they could put it on unlocked in plantarflexion ---knee flexion for our purposes---- and then go through an infinite range to the best she can do and keep her there. I've never used it, so I'm looking for feedback on that idea. Any other ideas would also be appreciated.
