Can anyone please offer any suggestions or recommend printed resources for
writing an initial appeal following denial of *L5686, addition to lower
extremity prosthesis, endoskeletal knee-shin system, microprocessor control
feature, swing and stance phase, includes electronic sensors, any type.*
Specifically the denial letter used the following language, it failed to
meet the following standards, that the device must be 'in accordance with
nationally recognized standards of medical practice, identified as safe,
widely used and generally accepted as effective… clinically appropriate… not
provided primarily for convenience… clearly substantiated and supported by
the medical records and documentation concerning the patient’s
condition…supported by the preponderance of nationally recognized peer
review medical literature.' Thanks in advance for any help.
