Dear Colleagues:
I am working on an interesting case and was wondering if anyone has any recommendations or previous experience with a similar patient.
This woman is 44 yrs old and states that she has been diagnosed with Genetic Muscle Spastic Paralysis. I have found some literature on Hereditary Spastic Paraplegia which describes the condition but no suggestions on effective treatments.
The patient ambulates with a cane and has been prescribed bilateral ASOs. She presents with muscle weakness in the lower extremities, left worse than right, affecting mainly hip extensors, knee extensors, plantar flexion and dorsiflexion. She also demonstrates poor proprioception bilaterally. The ASOs are for bilateral ankle instability.
She reports improvement in her gait after using the ASOs for several weeks with 1/4 lifts but finds them cumbersome in conjunction with footwear. We are interested in Orthotic treatment as well as possible E-stim use.
I appreciate any comments/suggestions.
