Dear list,
 
We are currently dealing with a 77 year old vascular client that has a skin adhesion directly over the anterior distal aspect of the tibia. The individual in question does very well on her prosthesis while in the hospital as an in-patient however, after being home for 3-4 weeks a small open soar has developed over the area mentioned above. This is the second time this happens. Socket volume is adaquat and there is no evidence of excess pressure in the problem area. She is currently wearing a ossur dermo locking liner with a SACH foot. They have tried various modalities (ultra sound, laser, physical manipulation, various creams, etc.) to try and eliminate the adhesion but nothing seems to work. I would appreciate any ideas on new socket designs, suspension methods or other treatment modalities that have been used in the past to deal with this type of situation. The individual in question also has severe arthritis in both hands which makes donning a suspension sleeve difficult.
