Hello,
Just received the list of replies from Mr. Rosenberg regarding his
brachial plexus injured client.
I just had a similar client in today.MVA nine years ago, fusion of
C5-C6. He had a trapezius transfer (never seen this before) in an
attempt to prevent subluxation of the humerus, and a latisimus dorsi
transfer to (down into the forearm) to give him flexion of the forearm
to horizontal. He has somewhat normal finger and wrist flex/ext. but
limited supination and pronation. This is the first complex brachial
plexus injury I have seen in my 9 years as an Orthotist.
He is quite functional but wants to prevent subluxation (hooks his thumb
on his belt to limit this a bit), and would like to start jogging and
needs to stabilize his arm at 90 deg and fix it to his torso. Of course
he wants something easy to don and doff.
I was looking at the Arm-adillo from New Options online, and this seems
quite adjustable and with some modifications it may do the trick. Do
you think I need to go to something more complex like a molded/padded
superstructure over both shoulders to distribute the pressure? He has
tried an Otto Bock shoulder support in the past and says it did not
prevent subluxation enough. Any ideas or pictures you could pass along
would be appreciated.
Thank you,
Derek Kozar M.Sc., C.O.(c)
