Hello list,
   
  I have an unusual case with a patient who is allergic to Postassium Chromate. This substance is used to tan most leathers. Long term use of leather footwear has caused him a severe dermatosis manifesting as continued weeping edema of the lower extemities which has made it almost impossible for him to walk at times. It has also lead to a widespread dermatitis from the severe inflammation of his feet. The referral asked for a vegetabel tanned leather boot with steel toes constructed so that he could continue to work in factory conditions. I have spoke with almost every orthopeadic shoe vendor that can be thought of. P.W. Minor came the closest to what were looking for with thier deer skin vegetable tanned leather. The patient cannot wear this kind of leather due to his work environment and that the Podiatrist will not allow a deerskin lining of the boot with a durable leather outer layer saying the socks will wick away moisture and with it the potassium chromate from
 the outer layer. The patient states he does fine in a pair of synthetic/man-made material shoes but they are not durable having to be replaced every month. This is an unusual quandry since the patients only option at this time is to replace the shoes every month, hence the need for a custom shoe. He has been to allergist's, podiatrists and physicians now to an orthotist. If anyone has any suggestions it would be greatly appreciated. I will re-post replies ASAP.
