I have an 8 yr old CP pt, currently wearing hinged AFO with PF stop. He is
a toe walker with mild crouching. The hinged AFO's were done at another
facility, so this is the first time I have seen him. During his evaluation
I noted that he is developing a rocker bottom on both feet, as his
achilles are quite tight and his activity level along with current bracing
has allowed the midfoot to stretch out. This is to the point where a
neutral looking ankle is actually 70-80% midfoot motion and 30-20%
subtalar motion. My feeling is he needs to move into a more rigid AFO
system that will halt further stretching at the midfoot and measures be
taken to reduce tone in the achilles. My concern is this level of bracing
will interfere with his activity level, i.e. recreational basketball. He
has no mental deficits and has excellent strength/ coordination of UE. Can
anyone confirm this my thoughts or offer a different perspective? Thnak you
