Occasionally a situation arises where we allow a BK amputee to home trial a
check/ test socket for a week. It is generally used to evaluate a major Px
design change, a different type of suspension, or a difficult fit. We
fiberglass wrap the socket connection and use an old foot & pylon from our
stock. We advise the patient verbally of the risks and explain that this is
not a definite socket. We document our instructions regarding daily usage
maximum minimums, use only indoors and use only with an assistive device. I
do, however, fear that eventually a fall will occur and the worst case
scenario will play out. Or perhaps not EVERYthing is properly documented.
Imagine that!
My question to the list is if anyone has any type of waiver form made up for
the patients to sign? If so, could you share yours? I will not use it
directly, but would like to see what others have listed or how these may be
formatted. Lastly, does anyone have suggestions as to how to make these
forms more viable or legal? Or do any of our national organizations have
such a form? Once I've made mine, I will get our attorney's advice as well.
