RT and LT modifiers

My billing person recently explained to me that she was advised by someone
at Medicare to use the RT and LT modifiers when billing things such as ankle
or knee joints. She uses 2 in the quantity along with the RT and LT
modifier to distinguish the RT and LT sides of the orthosis. My research
shows these modifiers as sides of the body and not sides of the orthosis.
I have searched online for nearly 2 hours for a definition of these
modifiers with no luck. If anyone can guide me to the specific section of
the manual that may provide any insight, I'd be grateful.

How do other facilities bill for joints on lower limb orthoses? Do you bill
quantity 2 and ONLY a LT modifier if it's a left brace? My billing person
told me that claims were continuously denied when she billed this way and
that was when she was advised to use both modifiers. This doesn't seem
correct to me.

Thank you in advance. Handling these Medicare denials in a new venture for
me.
