Dear colleagues,
I have a challenging patient- 32 yo female, K3 activity level, with elective left TTA due to osteomyelitic changes and recurrent ulcerations in lower limb, 7 months post-op.
History of Spina bifida affecting left lower-limb with quadriceps and hip extensor weakness. Pre-amputation mobility was independent with left KAFO. Currently independent without gait aids using primary interim prosthesis with silicone liner (invaginated and adhered anterior scarring) and clutch lock, multiaxial foot/ankle.
Problems: with continued gait training, patient's weakness is becoming more apparent with a 25 degree valgus at the knee joint on weightbearing, exaggerated by hip abductor weakness, presenting as 'knock-kneed' stance. I feel that the best way to control the valgus is through a thigh lacer but the patient is reluctant because of the associated heat and sweat issues, likening it to the cumbersome KAFO she previously wore.
This prescription is complicated by her plans to move to Papua New Guinea for a couple of years. I would be grateful for any suggestions of how to adequately control the knee joint, while reducing the heat and sweat issues in such a hot and humid environment. I will post a summary of responses.
