Hi all,
  We have been having problems with Medicare (is that a surprise?) regarding payment for shoes attached to metal braces. We have been billing with what I believe is the correct L code (L3224, L3225), with the RT and / or LT modifiers plus the KX modifier and they are routinely denied as non covered services. Occasionally we have to provide custom molded shoes when the patient's feet are so badly deformed that regular ortho shoes will not work. I don't believe that there is a specific code for custom shoes that has an allowable with Medicare (L3230, L3250, L3649 ??). Does anyone get these covered and if so, would you please share your secrets? Right now we have an angry physician because we have told the patient that he will have to pay for his custom molded shoes and, of course, he doesn't have the money..........
