Billing Question

Hello List
Has anyone seen language (esp. Medicare) that addresses if and how a claim
can be sent for a custom item that has not been delivered due to patient
non-compliance, e.g. multiple no shows, moved with no forwarding address, etc. At one
point I believe I saw information dealing with deceased patients but cannot
locate that either. It seems only fair that if the service includes evaluation,
casting, fabrication, fitting and follow-up, then a claim for that portion of
the service that has been completed should be sent. Any thoughts?
