I am hoping for some comments with regards to a scoliosis referral I
have received.
The patient is a 5 year old girl. She is diagnosed with spastic
quadriplegia brought about by Herpes meningitis at eight weeks of age.
She is now heavily disabled and a non ambulator. She has a marked
scoliosis with concavity to the right with Cobb angle of 69 degrees. She
has a dislocation of her right hip (suspected to be related to her
scoliosis) with limited right hip movement. She tends to lie in a
windswept position to the right and experiences discomfort when seated
in her wheelchair. With stretching she has clonus, an extensor
patterning and has an element of dystonia.
Her carers are concerned that an orthosis to address her scoliosis may
adversely affect her quality of life by limiting her torso movement.
These movements they believe to be the only controlled movement she has
from which she gains postural comfort.
Any comments with regards to the benefits (or disadvantages) of a TLSO
and related experiences welcomed.
