Dear List Members,

I'm working with a SCI patient who is wearing a horizontal cable RGO system with unilateral lateral uprights and plastic AFOs (with proximal/medial extensions). This patient is an adult male who weighs 200 lbs. The patient has done very well over the past few months of therapy; he can ambulate for extended distances with a walker, and even successfully negotiates ramps. He would like metal KAFO sections, because he is convinced that they will be easier to don independently. The manufacturer of the RGO discourages this, because of persistent breakage of the stirrups and a reduced toe lever (reducing posterior force at the knee, and making independent standing more difficult.)

What is your experience with metal KAFO/AFO sections on RGOs? Is this advisable?
