Listserve----I would appreciate your valuable suggestion on a difficult
case.

62 year female, BKA, 18 months out, no prosthetic care due to lack of
insurance coverage, amputation due to PVD. the difficulty is in her
measurements at the knee. She weight is 256 and a considerable amount
of tissue is concentrated in her thighs. 2 above knee center her
circumferential measurement is 8' larger than her knee center. She has
folds of tissue cascading down her thighs. Additionally her involved
extremity
exhibits 25 degrees of valgus with acceptable ROM. The contralateral
extremity is free of defect.

My first opinion was that she is not a candidate for prosthetic
treatment. I rapidly amended this when I asked her if she could stand
using a walker. When the walker was brought from the shed in the back
yard she promptly stood up without assistance and was able to stand
unassisted. You can imagine my surprise!

This lady deserves the opportunity to ambulate---even if at a K2 level.
With that being said I present you with the question:
in view of the Thigh development, her weight, the valgus at the
knee---how would you treat this patient prophetically?

Thanking you in advance,

Gordon Bass CPO, FAAOP
Mandeville, La.
