ORIGINAL POST
I'd like to hear your recommended L coding for a neuropathic walker
(CROW Boot) (Custom, plastizote lining, rocker sole, removable insole-
do you provide an extra insole?) I'll post responses. Randy McFarland,

RESPONSES
CROW boot: L1960, L2275, L2820, L2340, L3002, L3310 & L3400 *L3002,
L3310 & L3400 are not covered by Medicare
If patient is diabetic, use A5513 & A5503
Laced CROW boot: L1960, L2275, L2820, L2330, L2340, L3002, L3310, L3400
*L3002, L3310 & L3400 are not covered by Medicare
If patient is diabetic, use A5513 & A5503
L-1960 AFO, solid.
L-2350 PTB, If you incorporate this design.
If no PTB, then L-2340 (I've seen both 2340 & 2350 used on same device).
L-2820 Soft Interface (Some use quantity of 2 instead of 1). A-5513
Diabetic Insert, custom. If not diabetic, L-3020.
L-3400 Rocker Bottom.
L-2840 Fracture-type sock.
L1960 x 1, L2340 x 1, L2820 x 2, L2232 x 1, L2350 x 1 (if you go that
high), and L2275 x 1 (if there is a valgus or varus deformity).
Non-Medicare add L3010 x #. OR L4386 ;-) ;-) O:-)
The most common codes used for the the CROW walker are
L1960,L2340,L2280,L2275, and L3002( your removable insert). Variations
are
sometimes made when off loading or varus/valgus straps are added to the
orthosis or special rocker bottom soles. There have been inquires to the
billing of the rocker soles and it is usually rejected by insurance
companies if its set as an unlisted procedure code.
Solid ankle AFO
Custom anterior shell
Soft interface
L1960 L2232 L2340 L2820 L2999 (Similar to L3020 for use in total contact
AFO) only provide one, addL2350 if PTB design incorporated
(infrequently)
