Dear List Members,
I have a LBKA pt. with severe pain issues. I wonder if you could give
some direction.
Pt is otherwise healthy 40 yo male with 7 long BKA
This pt. started with a crush injury/ankle salvage, dealt with the pain,
finally amputated to BKA.
Pain followed up the limb, resulting pain reported to be of the same
intensity, implants for control attempted, but the wires were not in the
right location, waiting to try another implant. Pt cannot tolerate more
than a sock contacting his residual limb and is sensitive especially on
the distal end of both tibia and fibula, has tried Rx pain creams, seen
the PM&R, pain MD's, and has been through a custom liner and joint and
corset set-up with our office, but could not tolerate even the liner. He
can
withstand pressures at the knee, but not below the MPT, globally.
He has come to me with the feeling that the MD's are reluctant to explore
any other options but also
feels that possibly diagnostic surgery may reveal a solution.
My avenue would be something similar to providing an
ischial weightbearing, knee control, free below the knee type of
prosthesis/orthosis, basically a leg-type crutch to free up his hands
for ambulation, but the basic issue remains unsolved in dealing with the
pain.
