Our shear presentation is quickly approaching, and we are asking you all if
you could sacrifice a moment of your time and respond to our survey. We
have had limited response as of yet, and we value all of your opinions. We
have tried to make the survey as short as possible, so it shouldn't take
much time. We would like to thank those who have already participated and
those of you who are planning to.

This survey is being conducted by Dallin Chambers, Tim Schaefer and Robert
Weber, students in the Orthotic Practitioner program at Century
College in White
Bear Lake, Minnesota.

The goal of this survey is to learn how the orthotic and prosthetic industry
understands friction and shear, and what materials and techniques are used
to preserve orthotic and prosthetic goals, such as function, alignment, skin
integrity and comfort.

1. What is your affiliation to the O & P Profession?

_Practitioner _Technician _Doctor _Nurse _Patient _Other__________

2. Where do you live/practice?

3. How do you manage friction and shear?

4. What materials/tools do you use to manage friction and shear?

5. Where have you seen the negative effects of friction/shear in O & P?

6. What have been challenging factors in managing friction and shear?

7. What tools/materials would be helpful in managing friction and shear?

8. Do you focus on the prevention of the negative effects of friction in
your initial orthotic/prosthetic device design? If so, how?

We would like to thank all those participating in our survey. This is our
first venture in the field of research. We realize the importance and value
of research and publication in O & P, and we hope this will be a valuable
learning experience as well as a springboard for future projects. Any
suggestions, comments or advice is greatly appreciated.
