In an effort to further the Barr Foundation's mission in assisting amputees
who otherwise have no financial resources, the Barr Foundation and Taylor
Craft, Inc.,
would like to offer to qualified prosthetists (USA certified or state
licensed prosthetist participants only) our assistance for utilizing the
product trademark Frogskin.
The Barr Foundation will reimburse the purchase of the Frogskin kit for the
period designated as the Frogskin Pilot Program.
The Frogskin kit (only one per 'participating prosthetist') will be
available for a limited time at the discounted cost of $ 60.00
(normally $79 wholesale, plus DVD instructions, normally $ 21.95) to
pre-approved applicants and the participating prosthetists.
Shipping will be included at no additional cost for destination addresses in
the continental USA only.
Guidelines for Prosthetists Wishing to Participate in the 'Frogskin Pilot
Program' :
1) The prosthetist sponsoring an applicant that he/she deems qualified to
benefit from a retrofit to an existing socket must request an application
from the Barr Foundation. The sponsored amputee must not have alternate
funding resources and have the ability to provide proof through denial
letters from possible resources described in the application.
2) The 'participating prosthetist' will not charge the Barr Foundation nor
the recipient, nor any other sources, for installing this product for the
patient.
3) Once the application is approved, the Barr Foundation will authorize
Taylor Craft, Inc., manufacturers of Frogskin, to sell one kit at the $60
pilot program price for one 2.8 LB can (one kit can retro-fit three or four
adult BK sockets or approximately two AK 1/2 adults), and
the instructional DVD to the 'participating prosthetist' .
 4) The 'participating prosthetist' and patient will agree to hold harmless
the Barr Foundation and Taylor Craft, Inc., from any possible liabilities
of
the product or its installation.
 5) Prior to providing reimbursement to the participating prosthetist,
he/shewill complete and return the approved application
(with the photo of therecipient) and provide input to the Barr Foundation as
to the viability ofthe product and satisfaction (or lack of )
by both provider and the patient through written testimonials.The
participating prosthetist and patient agree that these testimonials can
be used at The Barr Foundation's sole discretion.
The Barr Foundation will then approve and send a $60.00 check to the
'participating prosthetist' as full reimbursement for the cost of the
product and cost of retro-fitting the socket.
 6) This offer is for a limited time only. We anticipate the product will
be well-received and endorsed due to the fact that it has
been utilized successfully by many providers across the country and abroad.

Notice: The Barr Foundation and Taylor Craft, Inc., (aka Frogskin) are
jointly underwriting this special offer in an effort to motivate US
prosthetists to assist
amputees with socket retrofits and who otherwise do not have the financial
resources to afford a new conventional socket design and fabrication.
 It has been emphasized pointed out in many recent discussions that a
loose-fit or a mis-fit socket can be experienced from the volume changes
and atrophy of theamputee's residual limb after regular use of the original
prosthesis. Anill-fitting prosthetic limb forces active amputees to
function in pain, if they can function at all, and increases the likelihood
of soft tissue injury to the residual limb.
For sufferers of poorly-fit or loose prosthetic sockets, Frogskin eliminates
the speculative procedure of chalking, sanding,and of adding extra ply.
The difficult alternatives of sloshing and rolling are no longer practical
with Frogskin on the market.
 
The Barr Foundation Amputee Assistance Fund was established in 1995 through
grants from The Barr Foundation.
The mission of the fund is to provide assistance to amputees who cannot
afford limbs, have no other financial
resources, and to promote quality prosthetic care for all amputees. This
program is a cooperative effort between 'the Fund' and the amputee's
prosthetist to improve the quality of life of the amputee.
The Barr Foundation has funded rehabilitation to over 1200 amputees through
438 sponsoring prosthetists.In order for the amputee to be considered
eligible for this Frogskin pilot program , they must contact a board
certified or state licensed prosthetist that is willing to evaluate and
sponsor them.
 The prosthetist, while considering whether to be a Frogskin pilot program
participant,must first evaluate the patient's needs, deem that the
Frogskin may be appropriate, and request the application directly from The
Barr Foundation.
 
The Application must be completed within 14 days and returned to us with a
$25.00 nonrefundable processing/application fee.
We will request that proof of denial of any other funding resources be
provided at the time the application is submitted.
The Application will be reviewed for the screening process by one or more
members of the review committee upon submission of the application, which is
to be completed by both amputee and prosthetist. The applicants may be
interviewed and will be considered for approval based on need, age, and
general health conditions.
The Barr Foundation will then have 2-3 weeks to process the application, and
the prosthetist will be notified as to approval or denial.
If the application is not returned completed within the 14-day period of
sending the application, it will be cancelled and the prosthetist
considering sponsorship
will have to resubmit. The socket retro-fit must be completed within three
weeks of receiving the approved application.
