It's been my experience that in severe PTTD (stage 3), where the navicular is on the floor, that full correction in an orthosis is not a good idea. To re-create the arch with the orthosis is usually too much for the patient to tolerate. I've seen and heard from some podiatrists, PT's and Orthotists that they take casts in a fully corrected position. I know that most people have surgery at a stage 3 collapse, but I have had several who don't want surgery or are not candidates.
What are your opinions on this? Have your severe PTTD patients traditionally tolerated full correction and if so, what orthosis were you using?
