I am seeing a 42 year old male patient with a med. length BKA due to trauma sustained 14 years ago. He has had no history of difficulty wearing a prosthesis until the last 18 months, approximately the same time that he gained 10 to 15 pounds. He now complains of extreme cramping in his posterior calf after walking no more than 100 feet. If he removes the prosthisis while cramping his calf appears tense and engorged with blood. He currently is wearing a hybrid PTB/TWB socket (posterior wall of socket is TWB shape) with an Alfa liner and a pelite liner in a suction socket with a knee sleeve. It is the same socket that he was wearing pain free for five years. I tried fitting him with a duplicate shaped check socket that I can experiment with altering every way imaginable (also altering alignment). I have had zero success efffecting the patient's symptoms. He has been to several doctors to eliminate the possibility of neuromas, bone spurs or circulatory problems. Any !
