Dear O&P Colleagues:
As I have done the last two years, I am offering the CMS Excel L-code file electronically, free of charge, to any who request it. I accept neither money nor responsibility.
This file is a combination of two files and some other information from CMS, to which I've applied my formatting. I have included both short and long descriptions, but the long descriptions I leave un-expanded (that is, they are still in one line, even if they are very long). I have added an average column to the right of the ceiling and floor columns and I have deleted all but L-codes, and even a few of the L-codes that I have no interest in. I have retained the fees for all states and regions. I would suggest that, to make this file most useful to you, you cut or copy your state's column and paste it over my average column. I have readjusted the Freeze Panes option and done other minor formatting.
That said, I hope you find this file helpful. Please request your copy of this file by email. That's the easiest way for me to make sure I have your correct address.
