The following information has been provided by the state. Be sure to apply
for license renewal, especially in light of what has been happening with CMS
and the recognition of licensed practitioners as the appropriate care givers
for O&P Medicare covered services in states with Licensure.









For the current Prosthetist-Orthotist profession with a license expiration
date of November 30, 2005, only 45.73% have renewed as of November 2, 2005.




For the current Prosthetist profession with a license expiration date of
November 30, 2005, only 38.75% have renewed as of November 2, 2005.



For the current Orthotist profession with a license expiration date of
November 30, 2005, only 40.63% have renewed as of November 2, 2005.



For the current Orthotic Fitter profession with a license expiration date of
November 30, 2005, only 21.05% have renewed as of November 2, 2005.



For the current Orthotic Fitter Assistant profession with a license
expiration date of November 30, 2005, only 24.60% have renewed as of
November 2, 2005.



For the current Pedorthist profession with a license expiration date of
November 30, 2005, only 35.53% have renewed as of November 2, 2005.



The Department of Health believes that more Prosthetist-Orthotists,
Prosthetists, Orthotists, Orthotic Fitters, Orthotic Fitter Assistants, and
Pedorthists plan to renew but may not have received their renewal notice due
to an incorrect address or may be delaying renewing until later this month.



To ensure a timely renewal, Prosthetist-Orthotists, Prosthetists,
Orthotists, Orthotic Fitters, Orthotic Fitter Assistants, and Pedorthists
may renew using MQA On-line Services by visiting www.DOH-MQAServices.com

Prosthetist-Orthotists, Prosthetists, Orthotists, Orthotic Fitters,
Orthotic Fitter Assistants, and Pedorthists until midnight, November 30,
2005.



MQA On-line Services account ID and passwords can be found directly on the
renewal notice. If the licensee did not receive a renewal notice, the
Department can provide the licensee with the account ID and password over
the telephone. To receive an account ID and password over the telephone, the
licensee should contact the Department at (850) 488-0595 and press menu
option 3. A member of the MQA Call Center can assist the licensee with this
information.
