In an attempt to better coordinate the requests for prosthetic donations and
supplies thru the Barr Foundation, we suggest the following.
Below are our guidelines for making requests, thru the Barr Foundation and
thru our Hope Donor members, Euro International , Alps South and others from
around the country whom have graciously offered assistance to Hurricane
Katrina amputee evacuees.
These are general guidelines set by the Barr Foundation and are required for
our participation in expediting and processing the requests and not
necessarily a requirement by any or all the manufacture donors , whom may
elect to work outside of our organization.
1) To better assure that the proper evaluation of the amputees needs is
safely conducted and the proper utilization of the specific components and
supplies are properly installed and provided by a qualified health care
provider, only ABC or BOC certified or state licensed prosthetist providers
can request prosthetic components and supplies from the Barr Foundation for
specific Hurricane Katrina evacuees amputees.
2) Qualified providers must agree not to charge the amputee or charge
third party payers for any of the prosthetic components/supplies donations
they receive and or for services they are providing if they utilize donated
supplies from us.
3) All requests for specific donated materials must be made via e-mail or
telephone to the Barr Foundation whom will determine and confirm the
professional credentials of the provider, and the name and profile of the
specific applicant.
4) A very short form application will be completed and quickly processed
for the manufacture Hope Donor to directly ship the specific item to the
provider or from the Barr Foundation warehouse, if in stock. Application
fees are waived for all qualified candidates.
