Subject: AK Question
I'm seeing an above knee amputee with a condition that is unusual and I could benefit from the ideas of others that have trod there before me.
The person in question is a 49 yo male of a muscular build who delivered marble slabs for kitchen counters for a living. He wears an AK endo prosthesis with a SNS unit and multi axis foot suspended by a heavy duty leather Silesian belt. His stump is conically shaped and is approximately 6 long. He started experiencing distal pain which I couldn't relieve so I sent him for x-rays. He has developed three antler like spurs at the distal femur that are ¼ to ½ in length that protrude posterior, medial and lateral. Any contact, even from pulling on a stump sock is painful. Surgery not an option for many reasons. A socket with a distal void did not stop the pain from the proximal pull of skin traction occurring with each step. With his heavy duty suspension belt the pistoning is minimal.
Since the process of rolling on a gel insert distracts the tissue distally relieving the bone spurs and that coupled with a distal strap from the insert to provide suspension for the prosthesis distracting the distal aspect of the stump further I'm thinking that this could be a combo that would be reasonable to provide relief from the bone spurs. I would like to hear from someone who has been there, done that.
