Call United Amputee Services Assoc. Inc They have a great booklet called
the Survivors Guide for the Recent Amputee
I was eventually able to get it for our patients at the VA hospital in
Houston and it has been received well. It doesn't cover everything you
mentioned but it is a good start and you could supplement it.
Call them for pricing and more info

Does anyone have a patient handbook for new amputees? Something they would
be given postoperatively which details: wound care, stretching, ROM
exercises, starter information on prosthetics, funding assistance, psych.
care, amputee support group contact info, etc.
We are trying to put one together in our hospital but was hoping other pt
care teams have one put together already that we may glean a little
assistance from in terms of what's important to include, what's too much
info., etc.
We really appreciate any help you could lend. I will email our mailing
address if you would like to share a copy with us.
