Dear colleagues,
I have a middle aged lady who had a CVA approximately 2 years ago and
has a consequent left hemiplegia. Functionally her left wrist / hand
control is recovering well, however she has developed contractures of
her long finger flexors that impede her functional ability. We are able
to passively stretch her wrist into reasonable extension with the
fingers flexed in her palm OR extend the fingers and MCP joints with the
wrist in flexion. However we are unable to achieve any useful extension
of both wrist and fingers / MCP joints together. She has very regular,
good quality physiotherapy and is not making lasting gains. Serial
casting has been tried but is not well tolerated and the results
achieved aren't carried over for too many days. Any suggestions
regarding a wrist / hand orthosis that might be useful (either prefab.
or custom) to achieve progressive extension of both the wrist and
fingers - preferably an option that will be able to be applied by the
patient herself.
