Hello, Here is my initial question and the responses seperated by an asterisk. Thank you for the responses.
Nathan Keepers, CPO
Which patella stabilizing knee orthoses do you like the best or think works the best?
I have a patient (my brother) who has had lateral subluxation problems for a long time and has used numerous types of orthoses for this. While wearing one, he dislocated it again recently. He has felt that the one with the removable patella support (palumbo I think) has been the best in the past. I have had good feedback from other patients on the Shields brace from Hely-Webber. It seemed like Bauerfiend had a hinged brace that had a good looking silicone support built in, is it any good?
Any thoughts appreciated.

It has been our experience that a good neopren ko with a superior buttress has worked best. Not felt but a good buttress with an open patella. The buttress should be a sewn in tube. Good luck. My tech uses them for ski jumping
*
Have you checked out the BREG PTO. Its a nice dynamic orthosis.

My name is xxxxxxx and I am a CO with Otto Bock Healthcare. My wife has a chronic problem with lateral subluxation with two instances of dislocation. We have used the Otto Bock Genu Patella Lux Knee Support with some success. The reason I like it is because it stays close to the knee center even in full extension. It seems that the braces that go further up and down the leg tend to bridge when the leg is in full extension. The brace part # is 7158. To determine the size you take a measurement directly around the knee, (not above or below). Call 800-328-4058 and press “1” for customer service.

Hi Nathan, I use the simple inexpensive one from Procare. It's a division of DJ Ortho. I get mine from Western Medical in Redmond, WA. It has a J butress with 2 pull straps. This one holds up well and seems to work as well as any.
*
I have used them all and tested them in my office with my patients by having the pt climb up and down steps with varying orthoses. The brace that consistently alleviates pain is the Tru Pull by Don Joy. The brace was first sold by Orthotech and was retained and repackaged when Don Joy bought Orthotec. It is the knee orthosis that I would apply to my mother
*
Strongest control I have found is the breg PTO. When the knee extends, the patella support increases. Much stronger than palumbo, hely-webber. Try one on yourself and you will see.
*
Well the reality is the silicone pad on the Bauerfind is not removable and if the patella is a chronic subluxer then even millimeters count, if the shields works well I would stick with it. The Breg has a brace called the PTO; however over the patella is a hard plastic piece which causes pain, so this is only helpful if someone subluxes on every step. The shields now has hinges if requested; however I do recommend removing the sewn in condyles if you go the hinge route.

I'm using Breg's Lateral J brace. My patients seem to like it. Be careful - the sizing seems to run small.

Palumbo = good, DonJoy Lateral J = good, Bauerfein sells direct to the docs and is very expensive=not good. Besides you don't need hinges for patella control.
*
In my experience, nothing works like a Breg PTO
*
I have tried them all! The only one that I found that provides any 'active' control for the lateral subluxation is the Shields Brace.
*
Nathan, I've always wanted to try this KO since it provides a semi-rigid lever arm to anchor to:

I have also used the Dynatrack in the past and find it to be much
better that the Shields brace:

I recently tried the Breg PTO(I think that is the abbreviation). It was a quite stable and easy to manage device that effectively control ed the patella instability. Call Breg and they will know just what you are looking for. Hope this helps.

You might also want to look at the PT Control K.O. from Medi.
*
My personal opinion is to use the patellar tracker from Breg. I put one
on one of our Rehab residents who is very active in soccor and she
loves it. She does not use it daily though, she uses it for activity and
uses a palumbo for day to day
