Medicare diabetic FOs

Hello List
I am evaluating the various ways we fabricate diabetic custom foot orthoses
with respect to the Medicare thickness specifications. Typically we begin with
a foam laminate of some kind and add a layer of cork for firmness and to be
ground in as posting, arch filler, etc. The cork does not extend to the end of
the foot. I am assuming that the foam laminate is the baselayer to which the
specification applies, and not the cork. Is this the conventional wisdom?
