Thank you to everyone that responded. Here are the responses as requested.

1) My question to you is how active is he at 365 pounds? I have a patient
who weighs 340 and has been on the total knee 2100 for years. No
problems at all. It is only rated to 275 though so just be aware of
that. My patient just walks. No impact sports. I hope this helps.

2) USMC Multiplex prosport is a hydraulic knee rated
to 350. You would need to get the patient to sign a waiver of some kind.

3) Try the prosport by usmc with a mauch unit. i believe it goes up to 350
and it may go higher. it has an extension stop as well.

4) Its a push but you can use a Seattle system Pro sport that uses the
hyperextension stop . sometimes you'll need to modify the stop to allow the
SNS to work / release better. They have other modular parts all rated at
350# but at this weight many people are just happy to ambulate and not in
the higher activities. Foot can be custom made by Otto Bock or Ossur.


5) There are no endo knees appropriate for that classification of patient,
yet!
Today, the exo Durathane knee system from Hosmer is your very limited
option. Ohio Willow Wood stopped making their exo system. 325# moderate
activity is not a problem. Mauch/Catech units will withstand 365# weight and
activity level but there is no frame available to withstand the stress and
torque. The pyramids are your weakest point as well as the pylon. If you fit
him with exo, it would be a great goal for him to lose the 40# and fit him
with a high tech leg.
