Thanks to all who responded. Here are all the responses thus far.

For lateral patella instability, we have had good success with Don Joy's
off the shelf patella stabilizer. I don't know about Peds. Breg's
doesn't hold up.
Could you let me know about your responses? My caseload is around 75%
pediatrics. I can find PSU's for teens (i.e. including my daughter), but not for
under that age. The 2 types of PSU's that fit kids are 1) Don Joy's 2-piece
Patellar Stabilizing Unit (I usually end up fitting the undersleeve at the
measured size and the smaller oversleeve one size smaller), these are availabe in
extra small; 2) DeRoyal has a nice wrap around/velcro version called the Patella
Stabilizer Concise, which would be easier to don and maintain fit during
volume fluctuations. DeRoyal's smallest x-small is for a 13 knee circumference.
Uriel make a range of Patella stabilising models and one that is quite small
and works very well on sports injuries it is also made small enough for
children, I do not remember the exact model or who distributes Uriel in the USA
We like the Hely-Weber “Shields Brace”
800-221-5465, they should get you some sizing info and brochure if you are
not familiar with it
I have not utilized them much but two other sources to check are:
1.R.J.Industries Ph.(800)-560-3255 and 2.Benik Ph.(800)-442-8910=can make
custome things well. Both Peds related.
All the kids I have fit with the Bauerfind Genutrain KO wear them out till
they are in shreds, and they come back for new ones or when they outgrow
them, so I assume they are well tolerated and acceptable to the docs
prescribing more of the same.
