Buck and everyone else, this language is taken directly from HR. 1.
 Like most people, the federal statutes use the term orthotics when
they mean orthoses.

(C) OFF-THE-SHELF ORTHOTICS- Orthotics (described in section
1861(s)(9)) for which payment is otherwise made under section 1834(h)
which require minimal self-adjustment for appropriate use and does not
require expertise in trimming, bending, molding, assembling, or
customizing to fit to the patient.

During the Negotiated Rulemaking Meetings, CMS staff had a very good
understanding of the difference between orthoses that came strait out of
the box and required no fitting expertise and orthoses that required the
bare minimum of a Registered Fitter. The key in the language is the
terms self-adjustment and trimming, bending, molding, assembling, or
customizing.
