Expecting an RX in the next day or two for a Minerva CTO to treat an atlanto-axial fx (odontoid?). Patient: young adult female, <100 lbs., developmentally delayed, hydrocephalic, incomplete quad (still breathing on her own), with a miniscule mandible (currently wearing SOMI mandible piece designed for 6 month to 1 year old).

Patient is in a facility in which specialized nursing care is spotty at best. If a CNA has to move mandible piece to facilitate feeding, it’s a proven fact that it won’t be donned again correctly.

Found a very old Minerva-like CTO buried in the shop basement that has posterior, medial and lateral coverage of head with forehead strap but no mandible piece. The name on it was ORTOPS CTO from Japan. Unfortunately it was too large.

Anyone heard of these folks?

Any thoughts of immobilizing the head without a chin piece?

I would prefer to tweek something off the shelf than to start from scratch, but if I have to make it, any suggestions?

Thanks for your help.
