Thankyou !!!
I would like to thank all the people who responded to my problem about
casting a patient with spasticity. The suggestions were very numerous. I will
list some of the ones that came up more than once.
1) Don't do it because even if you can get a cast the patient will never
tolerate the K.A.F.O's.
2) Go with something temporary such as KAFO's from measurements. If this
fails there is not a huge loss in time or $.
3) Have the doctor prescribe a drug that will temporarily put him out , or
reduce the spasticity long enough to cast him.
4) Reduce the spasticity during casting by casting in a supine position,
with the hip and knee bent to 90 deg. The foot/ankle should also be at 90.
5) do a 2 part cast by doing the AFO part first , and then the thigh
section.
6)Time the casting with some dental work when the patient will be under a
local.
7)Tell the doctor that you can't do anything until the spasticity is under
control.
I hope some of this helps others that may find themselves in this
situation.
