Good Evening All,

I would like the list to engage in a discussion about Gel liners and
Diabetic/ Vascular impaired patients.

I know that most of the liner manufacturers claim that the gel is a good
thing for protection ofr thin poor skin. The problem I have with that
thinking is that I have quite a few patients who will not heal once they
get a sore in a liner! My feeling for a long time has been that putting
a tight tourniquet type prosthesis on a diabetic limb is asking for
trouble.

I have had probably 4 different bk patients who have not kept up on
proper sock management, and have developed ulcers in varied locations on
their limbs. We keep doing adjustments, adding pads, even going to the
extent of casting and making a new GEL SOCKET. The sores would not heal,
even working with wound care center. Out of frustration I say Let's go
back to a pelite,sock, supracondylar,ptb socket. And one of them with
cuff strap and waist belt.(That particular one was 6 mo. with ulcer over
fib head, distal tib. In less than a month it was healed up. It has
stayed that way for over 6 mo.!) In every instance the ulcers have
healed in remarkable time.

Also how about diabetics who complain about aching in calf area when
using locking liner with pin VS. Gel Cushion liner with sleeve.

Interested in everyone's feelings on this subject . I will post
responses.
