I had great results with one client, he had severe psoriasis (transtibial in
my case). We met with the Dermatologist, he said that a total contact
silicone liner was the perfect solution when I made the recommendation to
him. He was correct, it cleared up the leg beautifully.

I have encountered two patients with psoriasis in the past year. They both
presented as transtibial and had suffered with psoriasis for some time. To
minimise the amount of shear at the skin/socket interface I provided them
with Alpha cushion liners with suction seal suspension. Both are doing
fantastically well and walking independently all day without any irritation
to their skin. The liners are lasting well and as long as the client follows
the cleaning instructions provided by Ohio Willow Wood I consider the Alpha
liner to be an excellent choice in this situation.

I have tried the Dermo and Stabilo liner from Össur on psoriasis with
very good success. The environment under the liner is good for the
psoriasis with the humidity and easy to clean. Use only medicament on
the residuum during the night!
If you need some picture to convince the dermatologist then I have some
and could send you.

Sore eye is... is what you get from reading too many letters on the list
serve!

The Dermo and Alpha will help due to:
1) Both will moisturize the skin which is very helpful with this skin
condition.
2) Both cover the skin and prevent air from reaching the lesions. This
effect actually has a beneficial effect on this disease.

I have experience with several psoriasis cases where I tried suction liners.
In most cases it did not work well. There would be episodes of improvement
only to be followed by a recurrence of the skin problems. Unfortunately,
even using basic systems and frequent sock changes did not prove much
better. While the psoriatic conditions remained chronic, there were a couple
of cases in which the dermatologist was able to help the condition of the
areas pertinent to the prosthesis with creams and medication to the extent
that the suction liner was tolerable. In the other cases, the only option I
was the use of less contemporary sockets and frequent sock changes. In other
cases the patients felt that the benefit of improved suspension outweighed
the detriment of increased discomfort, which in most cases they believed to
be minimal.

I'D TRY GIVING THE PATIENT A FEW SAMPLE SCRAP PIECES (2X2 SIZE) OF VARIOUS
BRANDS OF LINERS TO WEAR ON HIS SKIN AS A TRIAL TEST. HIS DERMATOLOGIST MAY
BE ABLE TO TELL HIM IF ALOE LOTIONS OR MINERAL OIL IS CONTRAINDICATED.
PERHAPS HE COULD TRY USING A PIECE OF STOCKINETTE TO ATTACH THESE STRIPS TO
HIS SOUND SIDE CALF OR WHEREEVER IT WOULD STAY ON FOR 8 HOURS OR SO (SINCE
THE OILS IN THE LINERS PREVENT MOST TAPE FROM STICKING) I HAVE BEEN USING
ALPHA LINERS ON PATIENTS WITH MILD PSORIASIS AND OTHER SKIN PROBLEMS BUT
NONE OF THEM HAVE BEEN SEVERE CASES. GOOD LUCK!
