Thanks to all who responded. This gives me some ideas to work with.
I¡¯m needing the offset or slider to inset a Springlite luxon max dp foot
to allow a plantargrade foot and at the same time allow proper lock
placement. The pt has a fleshy residual limb, so the long pin is
necessary in combination w/ a clutch lock.

Try using Otto Bock's sliding adaptor 4R103. It works with some pins,
but I've found it allows the longest pins. Another practitioner here
has put that unit at the foot attachment, but I don't believe that is
recommended by the manufacturer.

Dear Mr. Harvey,
I don't think that there is any one the market, my suggestion is that
you use the ?sur Offset adapter and take the risk of drilling a hole
through it!! That is not a good advice but the only one that I could
come up with. If this is because of un-stabilized soft tissue then way
not use the lanyard cord instead?

try PDI offset plates, they have a hole in the center that tends to line up with the pin.

If you use a 'grace plate' in your lamination or vacuum forming process
you
can adapt any four-hole pattern apparatus. You gain chamber depth for
the
longer pin by simply adding spacers (made from polypro or prefabricated
aluminum) between the finished material under the plate and the
four-hole
apparatus. Be sure to insert screws into grace plate attachment holes
before
laminating or pulling material so that when you back them out there
will be
chambers for the attachment screws. Also, make sure you insert the
lamination dummy into the grace plate to create the channel for
screwing the
locking mechanism (I use Fillauer) into the distal socket.
