As a couple of questions have already been posted I thought I would offer some information on this subject:
CMS provides fees for many of the L codes.
In some cases the DMERCs have fees that seem consistent for many of the other codes.
If you want to get these allowables( for lack of another term) they can be obtained by writing to the Freedom of Information Branch for the respective DMERC.
I am in region B and at last publishing (97-03 Bulletin) the address was
Freedom of Information
P. O. Box 50454
Indianapolis, IN 46250-0454
If anybody has a better address, or the address for the other DMERC's FOI branches that would likely be helpful information to the rest of the list.
You need to send them a list of codes, and the respective states for which you want information.
If people want to return the other address to me I will post.
