Text:
The deadline for your application as an Illinois licensed orthotist,
prosthetist or pedorthist using either Option I or Option II of the
grandfathering rules is less than two weeks away. The Department of
Professional Regulations must receive your application and check by 1-1-02
or you will not be eligible to use grandfathering to receive your
license(s). The four page Application for Licensure and/or Examination
form (IL486-1019 02/01 [LT]) and the Work History (IL486-1071 9/99 [LT])
must be received to start your file for each license that you apply for.
Other document mailed by ABC, BCP or physicians may be recieved after the
deadline but that will delay the receipt of your license.

information.
