Thanks to all who replied. I did get several leads on Italian O&P, as well as opinions on extension assist devices for knee orthosis applications. The general consensus is that a light, short patient/client is best suited for the forces involved. Unfortunately this patient/client is neither of those. Some component sources are listed below:
Scott Orthopedic Labs. Pre-fab KO with rubber tubing extension assist
Becker Orthopedic - Trick Knee hinge
Otto Bock - Post. offset with rubber O-ring extension assist. Highly endorsed by one practitioner.
Also suggested were various configurations of elastic straps etc used on standard K.O.s. In this case we will continue to use posterior offset hinge on a KAFO with floor reaction from the dorsi stop ankle. That will assist in the effort to resist distal migration, if you know what I mean. Everyone have a great summer
