Dear Lit Serve Members:
I am one of a group of seven George Brown College Prosthetic/Orthotic
students who are working on developing a Canadian standardization for
clinical note taking in our field. We are busy collecting as many different
assessment forms from as many different clinics as we can in order to give
us an idea of what clinicians are currently recording. If it is as all
possible we are asking you to fax us your current clinical assessment forms
to 416-480-5975, Att: Gord Ruder C.O.(c). We would also welcome any
suggestions as to what you would like to see be included in the list of
criteria for clinical note taking. Any comments can be directed to me by
