You have, in two paragraphs explained the danger and the real victims
of inside discounting and leveraged competitive bidding!
The patient recieves less coverage, and fewer choices of providers.
The independent may be forced to close his doors.

Hi Tony-
It's a bit frustrating for the independents to have to compete with the
large companies who also own manufacturers. When the products are sold at
cost to their own offices, it artificially lowers their cost of sales
allowing lower than realistic discounts to be offered. When Medicare does
an audit and sees their costs, what will Medicare most likely do to
allowable rates for our industry? Nothing good, for sure.
This false cost-savings comes out in wash eventually, but really is
just atemporary trick to encourage the chain offices to use their product
whice
the independent offices who continue to patronize the large company's
distribution branch, end up subsidising the manufacturing branch, the
chain offices and of course the large company itself.
