This is an interesting question.
Just off the top of my head, perhaps the question should be broken down to:
1)Fundamental appropriateness of early post op fitting in this situation
2) Fundamental appropriateness of early post op fitting without adequate
supervision, monitoring, and training.
3)Did the suture line dehiss and get infected because of inadequate healing
and premature full weight bearing---(which is probably covered by #2)
4) Whether the choice of the Iceross would be any more or less appropriate
than another system is another question.
Are the distal lesions from inadequate padding and pistoniong, is the distal
end being milked because of pistoning, would a gel liner where pistoning
was elimninated have been appropriate because of fragile skin?
5) Would any prescription be appropriate here such as the Air Limb, the post
op system carried by TEC, the Flo-Tech system, or the early weight bearing
casting protocols that Dr Burgess popularized?
Good question! I'll will be interested in others comments as well.
Might be important to get a definitive time line on dates of amputation,
status of wound healing when fitted, degree of supervision and personnel
involved, experience of prosthetist in early post op principles and follow
up, and the involved surgeons perspective and experience with immediate post
op fittings.
