Dear List Mates,
In collaboration with Dr. Dudley Childress at Northwestern University's
Prosthetics Research Laboratory and Rehabilitation Engineering Research
Program, my colleagues and I are developing an outcomes data base to
describe (1) client characteristics, (2) functional status, (3)
health-related quality of life, and (4) satisfaction with services and
equipment. Our goal is to develop a data base that allows clinics to track
the outcomes of clients. We have completed one field trial and are in the
midst of a second field trial. I am writing with two requests:
1. I would very much appreciate receiving copies of forms being used to
document any of these characteristics. In particular, we would like to
capture client characteristics such as amputation, disability, prosthesis
provided, orthosis provided, complications, etc. Clinic registration
forms, prosthetist or orthotist chart forms would be helpful.
2. Would any one be willing to comment on and critique the forms we have
developed? In particular, the client characteristics form needs some
shortening and filling in of gaps, despite our efforts to seek feedback.
