Many thanks to all those who responded to my request for insights into the
nature of our client's situation. The consensus was three-fold.
a) trapped interstitial fluid being forced out hydrostatically.
b) a gel liner (most recommended was the Alpha) would help
c) increase the distal pressure.
I suppose it would have been worth mentioning that this is an Ontario
situation, meaning that money for such items as alpha liners and flex feet
is not available. That is one reason I chose to use a single axis foot (I
was mistaken in saying that I used a sach). My choice of a Silipos gel sock
was my cost-effective substitute for an expensive liner. As far as distal
pressure goes, I have used an end pad made from prosthetic end pad foam
(doesn't THAT clear it all up) available from Orto Ped.
My result to date is that he has had no problems during his admittadly
limited periods of weightbearing. The medical end of the team suggests
restricting him to limited prosthetic use until his skin toughens to contain
his fluid volume better. I suppose we'll see over time how well the
combination works.
