The device used to be available from Fillaurer in Chattanooga, TN. The
procedure can be found in old NYU and UCLA text pages. These were usually 3
ring binders of course materials given to students that attended the
specified prosthetics course.
Best of luck! If forced to I may be able to dig one out from the storage.
Hope you can get one from another source.
