Hi John:
Please find attached a pre-prosthetic evaluation form designed by Gertrude
Mensch, PT, MCPA and myself CPO(D) with a lot of help of US colleagues
(CPO's). The attachment is in MS word 97 and can be sent in other versions,
should you have any problem opening this one or not be compatible with your
programs.
The condition under which I send this to you, (or anyone else) is that
you'll give me some feedback on your experience and that, sticking to
copyright considerations, you do not publish this under your name.
If you intend to use it, please confirm.
