I hope this information is useful to you.
As far as to how providers are notified of policy changes there are several ways. The most common and fastest way is that each remittance voucher they receive with their payments contains information in it. It is in these vouchers that late breaking news is included. The information about the DME changes has been running now for several weeks at least.
The second way is sometimes letters are sent directly to the affected providers. I'm not sure that this happened in this instance. Finally, new handbook pages are sent as soon as they are published.
