The best elbow padding I've found is the Spider Pad by DonJoy. It has many
small pads in a honeycomb configuration that wraps around both the
epicondyles and the olecranon. You can probably use the off-the-shelf
version (which is about $14) or they can custom make one for you. I have
used both Bauerfeind and Silipos and a number of other types and It's the
best I've found.
